Clinic Billing Basics: Packages, Discounts and Clear Receipts

Unclear clinic bills lead to disputes and lost trust. How small clinics should structure consultation fees, procedure packages, discounts and receipts so patients understand exactly what they paid for.
Patients do not mind paying for good care. They mind surprises: a bill higher than expected, a package that did not include what they assumed, a discount given to someone else. A clear billing structure protects your reputation as much as your income.
Structure your fee list
| Type | Example | Tip |
|---|
| Consultation | First visit, follow-up within X days, follow-up after X days | Write the follow-up window clearly |
| Procedures | Dressing, injection, minor procedure | Fixed rates where possible |
| Packages | Physiotherapy sessions, dental treatment plans, antenatal care | List exactly what is included and excluded |
| Diagnostics | In-house tests | Price per test; packages for common combinations |
| Medicines (if dispensed) | Billed separately | Batch and expiry on the bill where applicable |
Packages: write down what is included
Most disputes come from packages. Every package should state:
- Number of sessions or visits
- Validity period
- What is included (and what is not, such as medicines, tests or implants)
- Refund rules if the patient stops partway
Give the patient a copy at the start.
Discounts: have a policy
Ad-hoc discounts create unfairness and confusion. Set categories (for example, senior citizens, staff families, camps) with fixed percentages, and record the reason on the bill.
What every receipt should show
- Clinic name, address and registration details
- Patient name and date
- Each service with its amount
- Discount and reason
- Total paid, payment mode and balance (for packages)
- Doctor's name where relevant
Send a copy on WhatsApp or email if the patient wants a digital receipt for records or insurance.
Taxes
Many healthcare services by clinical establishments are exempt from GST, but some services or items may not be. Ask your CA how your services and any products you sell should be billed.
